Primary Function:
Provides case management, coordinates services and counsels CHCB patients.
Duties and Responsibilities:
Counsels patients on a short-term basis and/or performs crisis intervention services.
Assesses patient suitability and assists patients with obtaining and completing forms for entitlements, supplemental insurance and other needed benefits that will help eliminate dependence on indigent and emergency assistance programs.
Assists patients in accessing prescription cost assistance through indigent programs sponsored by pharmaceutical companies and community resources.
Acts as an advocate for patients when interacting with community agencies.
Maintains up-to-date and accurate patient records. Prepares reports, records and other written documentation as needed.
Coordinates patient services, such as transportation assistance, when necessary. Prepares patient letters when applicable.
Identifies need for patient education and support services, such as pregnancy counseling, parenting skills, caregiver skills, etc.
Consults with Providers and makes appropriate referrals when necessary.
Evaluates needs of the special population patients, including the homeless, substance abusers, battered women and at-risk adolescents.
Provides HIV pre-test counseling and testing as needed. Refers Positive patients for post-test counseling.
Follows-up on missed appointments with patients who appear to have a pattern of excessive cancellations or failure to show for appointments.
Assists with patient flow including registering patients, answering telephones, and making appointments as needed.
Maintains a clean, safe, orderly work area.
Attends clinical and team meetings as needed.
Regular attendance is required.
Performs other relevant duties as assigned or requested.
Physical Demands: Requires periods of sitting, standing, walking, bending and stooping. Weekend and evening work may be required.
Salary Status: Hourly; non-exempt.
Minimum Qualifications: Bachelor’s Degree in Social Work or related field. Two (2) years experience as a Case Manager with strong interpersonal, oral and written communication skills. Computer literacy. Proficiency in MS Office.
Preferred Qualifications: Five (5) years experience as a Case Manager. Experience working with indigent patients.
