**Job Summary:**
The Mental Health Counselor (LMHC) will be responsible for performing case management and counseling activities related to the bio-psychosocial needs affecting the health and well-being of clients. The role involves assessing clients' psychosocial needs, providing short-term counseling, crisis intervention, and coordinating various support services.
**Responsibilities:**
- Conduct psychosocial assessments to determine appropriate treatment and referral needs for clients.
- Assist clients in accessing entitlements, supplemental insurance, and community resources to reduce reliance on emergency assistance programs.
- Advocate for clients when engaging with community agencies and organizations.
- Maintain accurate client records, prepare reports, and documentation as necessary.
- Coordinate transportation assistance for clients as needed.
- Attend clinical and team meetings, participate in case conferences.
- Identify educational and support service needs for clients, such as pregnancy counseling, parenting skills, and caregiver support.
- Conduct need assessments and education for specific client populations.
- Collaborate with healthcare providers and make referrals as required.
- Provide counseling and testing for HIV and other health-related services.
- Follow up on missed appointments and ensure continuity of care for clients.
- Contribute to Quality Improvement and Quality Assurance activities.
- Maintain a clean, safe, and organized work environment.
- Attend mandatory meetings and fulfill job-related duties as assigned.
**Qualifications:**
- Licensed Mental Health Counselor (LMHC). NYS Permit holders are also encouraged to apply.
- Minimum of three (3) years of experience as a Mental Health Counselor preferred, with excellent interpersonal and communication skills.
- Preferred experience with trauma-informed care and EMDR Therapy.
- Proficiency in MS Word, Excel, and Outlook.
- Ability to manage physical demands including sitting, standing, walking, bending, and occasional weekend and evening work requirements.
**Salary Range:**
$53,000 - $60,000 annually
Sponsorship of H1B Visa will be considered.
