**Job Summary:**
The Community Health Center of Buffalo, Inc. (CHCB) is seeking a Licensed Master Social Worker (LMSW) to provide counseling services and case management to address the bio-psychosocial needs of patients. This role plays a crucial part in delivering high-quality healthcare services to the community.
**Qualifications:**
- Master's Degree in Social Work or a related field.
- Licensed Master Social Worker (LMSW) certification in New York State.
- Minimum of two (2) years of relevant Social Worker experience; five (5) years preferred.
- Strong interpersonal, oral, and written communication skills.
- Proficiency in MS Word, Excel, and Outlook.
**Responsibilities:**
- Offer case management and counseling services to address patients' bio-psychosocial needs.
- Evaluate patients' psychosocial requirements and recommend suitable treatment and referral options.
- Aid patients in completing necessary forms for entitlements, supplemental insurance, and other benefits.
- Advocate for patients in interactions with community agencies.
- Maintain accurate patient records and coordinate transportation assistance.
- Attend case conferences, team meetings, and provide patient education.
- Collaborate with healthcare providers, make referrals as needed, and evaluate special population patients' needs.
- Provide HIV pre-test and post-test counseling and testing, and follow up on missed appointments.
- Contribute to Quality Improvement and Assurance activities while ensuring a safe work environment.
**Physical Demands:**
This role requires various physical activities such as sitting, standing, walking, bending, and stooping. Occasional weekend and evening work may be necessary.
**Salary Range:**
$57,262 - $67,100 per year
