Licensed Clinical Social Worker

Department: Behavioral Health
Location: Buffalo, NY

The Community Health Center of Buffalo (CHCB) is growing and seeking an experienced LCSW to join the team! CHCB offers a competitive compensation and benefits package including medical, dental, vision, life insurance, 401k match, generous paid time off, loan forgiveness (if applicable) and tuition assistance.

Primary Function:

Performs case management and counseling activities related to bio-psychosocial needs affecting the health and well-being of Center patients.

Duties and Responsibilities:

  • Assess patientís psychosocial needs and determines appropriate treatment/referral needs. This might include case management short-term counseling and/or crisis intervention.
  • Assess and assist patients with obtaining and completing forms for entitlements, supplemental insurance and other needed benefits that will help eliminate dependence on indigent and emergency assistance programs.
  • Assists patients in accessing prescription cost assistant through indigent programs sponsored by pharmaceutical companies and community resources.
  • Acts as an advocate for patients when interacting with community agencies.
  • Maintaining up-to-date and accurate patient records. Prepares reports, records and other written documentation as needed.
  • Coordinates patientís transportation assistance when necessary.
  • Attends clinical and team meetings related to case conferences.
  • Identifies need for patient education and support services, such as pregnancy counseling, parenting skills, caregiver skills, etc.
  • Conducts need assessments and education for all new, ongoing prenatal and post-partum patients.
  • Consults with the Provider and makes appropriate referrals when necessary.
  • Evaluates needs of the special population patients, including the homeless, substance abusers and at-risk adolescents.  
  • Provide HIV pre-test, post test counseling and testing as needed.
  • Follow-up on missed appointments with patients who appear to have a pattern of excessive cancellations or failure to show for appointments.
  • Participates in and contributes to Quality Improvement and Quality Assurance activities and functions as needed.
  • Maintains a clean, safe, orderly work area.
  • Attends meetings as required.
  • Regular attendance is required.
  • Performs other relevant duties as assigned or requested.

Physical Demands: Requires periods of sitting, standing, walking, bending and stooping.   Weekend and evening work may be required.

Qualifications: Licensed Certified Social Worker (LCSW). Three (3) years experience as a Social Worker with strong interpersonal, oral and written communication skills. Computer literacy. Proficiency in MS Word, Excel and Outlook.
