Job Summary:
The Health Educator will coordinate project activities, provide patient education, conduct chart reviews, and follow-up at the Community Health Center of Buffalo, Inc. (CHCB). In this role, you will develop and implement culturally tailored educational programs with community-based organizations serving low-income underserved populations. Responsibilities:
- Educate patients on cancer screening, treatment, and follow-up importance.
- Conduct chart reviews to identify care gaps and recommend solutions.
- Develop and implement programs to increase cancer screenings, with a focus on colorectal, cervical, and breast cancers.
- Participate in quality improvement activities and evaluate care quality.
- Collaborate effectively with a multidisciplinary team.
- Communicate efficiently with patients, families, and team members.
- Coordinate activities to enhance cancer screening with the interdisciplinary team.
- Ensure program outcomes align with goals and address any variances.
- Perform other relevant duties as assigned.
Qualifications:
- New York State RN License with a minimum of 3 years of experience in a primary care setting.
- Current CPR certification.
- Experience in coordinating and providing educational programs to patients.
- Ability to foster positive business relationships.
- Proficiency in collaborating with various colleagues and functioning within a diverse care team.
- Computer literacy, specifically in Microsoft Office.
Physical Demands:
The role involves sitting, standing, walking, bending, and stooping. Some evening work may be required.
Salary:
$68,000 per year
