Job Summary:
The Health Educator will play a vital role in coordinating project activities and providing patient education, chart review, and follow-up at the Community Health Center of Buffalo, Inc. (CHCB). As a Health Educator, you will also conduct culturally tailored educational programs with community-based organizations that serve low-income underserved populations. Responsibilities:
- Educate patients about the importance of cancer screening, treatment, and follow-up.
- Conduct chart reviews to identify screening, treatment, and referral gaps and make recommendations for closing the care gap.
- Develop and implement on-site and community-based programs to increase cancer screenings, specifically for colorectal, cervical, and breast cancers.
- Participate in quality improvement activities and the measurement and evaluation of quality-of-care measures.
- Function effectively as a member of a multidisciplinary team.
- Consistently demonstrate effective communication with patients, families, and team members.
- Coordinate activities to improve cancer screening in collaboration with members of the inter-professional interdisciplinary team.
- Ensure desired program outcomes are achieved and address variances from established goals as necessary.
- Perform other relevant duties as assigned or requested.
Qualifications:
- New York State RN License with at least 3 years of experience working in a primary care setting.
- Current CPR certification.
- Demonstrated experience in coordinating and providing educational programs to patients.
- Proven ability to build positive business relationships.
- Ability to collaborate effectively with colleagues, including clinicians, pharmacists, social workers, etc., and function as a member of a diverse multidisciplinary care team.
- Computer literacy and proficiency in Microsoft Office.
Physical Demands:
This position requires periods of sitting, standing, walking, bending, and stooping. Evening work may be required.
Salary:
$68,000 per year
