Community Health Worker

Department: Prevention & Wellness
Location: Buffalo, NY

**Job Summary:**

The Community Health Center of Buffalo, Inc. (CHCB) is seeking a part-time Community Health Worker to join their mission-driven team at a state-of-the-art health center. The Community Health Worker will collaborate with community groups to identify patients in need of screenings and provide culturally tailored education, navigation assistance, and non-clinical instructions to address barriers to care.

**Responsibilities:**

- Provide culturally tailored health promotion and education on cancer risks.
- Assist patients in accessing and navigating community resources to increase cancer screenings.
- Help individuals obtain appropriate cancer screenings and medical services.
- Educate patients and families on the benefits of cancer screenings and risks of developing cancer.
- Ensure patients receive timely services and referrals.
- Support patients in adhering to medical plans of care, preventive screenings, interventions, and follow-up appointments.
- Participate in community outreach events to promote cancer screenings.
- Document program/workshop/service information accurately.
- Attend program-related meetings, presentations, and outreach activities.
- Cultivate relationships with external agencies, community groups, and professionals.
- Develop effective community outreach procedures.
- Prepare necessary reports and documents.
- Perform other assigned duties.

**Qualifications:**

- High School Diploma or GED required.
- Completion of Medical Assistant training.
- At least 3 years of experience in a community setting with low-income populations, linking individuals to services.
- Strong oral and written communication skills.
- Ability to interpret and apply written guidelines.
- Proficiency in MS Word, Excel, Outlook, and database programs.
- Physical ability to stand, walk, bend, and stoop.
- Willingness to work weekends and evenings when needed.

**Salary:** $19.23 per hour.
