Community Health Worker

Department: Prevention & Wellness
Location: Buffalo, NY

The Community Health Center of Buffalo, Inc. (CHCB) is searching for a part time Community Health Worker to join our mission-driven, well established, state-of-the-art health center.

Primary Function:

The Community Health Worker will work directly with community groups to identify patients overdue for screening and work collaboratively with the Nurse Educator to provide culturally tailored education and navigation that addresses barriers to care (e.g., transportation, childcare, interpreter services, insurance coverage). The Community Health worker will also provide appointment reminders, and non-clinical instructions for screenings.

Duties and Responsibilities:

  • Provide culturally tailored health promotion and health education on cancer risks.
  • Increase colorectal, cervical, and breast cancer screening by assisting patients and families to access and navigate community resources and services.
  • Assist community members to obtain appropriate cancer screening and medical services.
  • Help patients and families understand the benefits of cancer screening and the risks of developing cancer.
  • Ensure patients receive appropriate and timely services and referral.
  • Assist patients through support and referrals to ensure adherence to the medical plan of care including all appropriate preventive and disease-specific screenings, interventions, treatment goals (including self-management goals) and follow-up appointments.
  • Reinforce culturally appropriate health education.
  • Support team goals through an integrated approach to care management and community outreach.
  • Conduct at a minimum monthly community outreach events to increase cancer screening.
  • Documents all necessary information for programs/workshops/services provided.
  • Participates in scheduled program-related meetings, presentations, and outreach activities.
  • Develop and maintain relationships with external agencies, community groups, support groups, and other related professionals.
  • Implement process and procedures for effective community outreach activities.
  • Prepare reports and documents as needed or requested.
  • Performs other duties as assigned.

Physical Demands: Requires periods of sitting, standing, walking, bending, and stooping. Weekend and evening work may be required.

Minimum Qualifications: High School Diploma or GED. Medical Assistant training with at least 3 years working in a community setting with low-income populations providing resources and links to services and programs for patients, staff, and other agencies. Ability to effectively communicate orally and in writing. Ability to interpret, adapt and apply written guidelines and procedures. Computer literacy with a working knowledge of database programs and spreadsheets. Proficiency in MS Word, Excel, and Outlook.

Pay: $19.23/hr.
