Social Worker

Department: Behavioral Health
Location: Buffalo, NY

Job Summary:
The Community Health Center of Buffalo, Inc. (CHCB) is seeking an experienced and compassionate Licensed Social Worker to join the team. CHCB is a Federally Qualified Health Center, committed to delivering high-quality healthcare services to the community. Will be responsible for providing counseling services and case management to address the bio-psychosocial needs of patients.

Qualifications:
- A Master's Degree in Social Work or a related field is required.
- A Licensed Certified Social Worker (LCSW) certification is preferred.
- Must have a minimum of two (2) years of experience working as a Social Worker with strong interpersonal, oral, and written communication skills; five (5) years of experience is preferred.
- Proficiency in MS Word, Excel, and Outlook is necessary.

Responsibilities:
- Provide case management and counseling services to meet patients' bio-psychosocial needs.
- Assess the psychosocial requirements of patients and determine suitable treatment and referral options.
- Assist patients in completing forms for entitlements, supplemental insurance, and other benefits.
- Advocate for patients when engaging with community agencies.
- Keep patient records accurate and current.
- Coordinate transportation assistance for patients as needed.
- Attend case conferences and team meetings.
- Identify the necessity for patient education and support services.
- Conduct need assessments and education for prenatal and postpartum patients.
- Collaborate with the healthcare provider and make referrals when appropriate.
- Evaluate the needs of special population patients.
- Offer HIV pre-test and post-test counseling and testing.
- Follow up on missed appointments with patients.
- Participate and contribute to Quality Improvement and Quality Assurance activities.
- Maintain a tidy and safe work environment.
- Fulfill other relevant duties as assigned or requested.

Physical Demands:
This role involves periods of sitting, standing, walking, bending, and stooping. Weekend and evening work may be necessary.

Salary Range:
$56,181 - $64,522 per hour.

This position is represented by 1199 SEIU United Healthcare Workers East.
