**Job Summary:**
The Community Health Center of Buffalo is seeking an experienced Family Planning Nurse Practitioner to provide outpatient care to patients in a modern facility. **Responsibilities:**
- Adhere to all clinical policies and procedures, including infection control and confidentiality.
- Assess physical and psycho-social health status through medical history and examination.
- Order and interpret laboratory tests.
- Triage, diagnose, and treat illnesses and physical conditions within the specialty.
- Perform therapeutic measures, prescribe medication, and provide preventive health maintenance and education.
- Teach, counsel, and educate patients, families, and groups.
- Participate in quality improvement activities.
- Comply with collaborating physician guidelines and regulatory agency requirements.
- Perform special assignments as needed.
- Prepare documentation, maintain records, and ensure information protection.
- Fulfill other relevant duties as assigned.

**Minimum Qualifications:**
- Licensed NYS, board-certified, Registered Professional Nurse Practitioner.
- Experience as a Family Planning Provider, including Long-Acting Reversible Contraceptive.
- Computer literacy with proficiency in MS Word, Excel, and Outlook. Experience with eClinical Works is preferred.

**Salary Range:**
$103,000 - $108,000
