**Job Summary:**
The Licensed Mental Health Counselor (LMHC) will be responsible for conducting psychosocial assessments, providing counseling, crisis intervention, and coordinating support services for clients. This role involves advocating for clients, maintaining accurate records, and collaborating with healthcare providers to ensure the well-being of clients.
**Responsibilities:**
- Conduct psychosocial assessments to identify treatment and referral needs for clients.
- Assist clients in accessing community resources and entitlements to reduce reliance on emergency assistance programs.
- Advocate for clients when interfacing with community agencies and organizations.
- Maintain detailed client records, prepare reports, and documentation as required.
- Coordinate transportation assistance for clients as necessary.
- Participate in clinical meetings, case conferences, and team discussions.
- Identify educational and support service needs for clients, such as counseling, parenting skills, and caregiver support.
- Collaborate with healthcare providers and facilitate referrals when needed.
- Provide counseling and testing for various health services, including HIV.
- Ensure continuity of care by following up on missed appointments.
- Contribute to Quality Improvement and Quality Assurance initiatives.
- Maintain a safe, organized work environment and attend mandatory meetings as scheduled.
**Qualifications:**
- Licensed Mental Health Counselor (LMHC). NYS Permit holders are also encouraged to apply.
- Minimum of three (3) years of experience as a Mental Health Counselor preferred.
- Excellent interpersonal and communication skills.
- Experience with trauma-informed care and EMDR Therapy is desirable.
- Proficiency in MS Word, Excel, and Outlook.
- Ability to meet physical demands (sitting, standing, walking, bending) and occasional weekend or evening work requirements.
**Salary Range:**
$53,000 - $60,000 annually
