Licensed Master Social Worker (LMSW)

Location: Buffalo, NY

Job Summary:
The Community Health Center of Buffalo, Inc. (CHCB) is seeking an experienced and compassionate on-site Licensed Master Social Worker (LMSW) to join the team. CHCB is a Federally Qualified Health Center dedicated to providing high-quality healthcare services to the community. The Social Worker will be responsible for offering counseling services and case management to address the bio-psychosocial needs of patients.

Qualifications:
- A Master's Degree in Social Work or a related field is required.
- Licensed Master Social Worker (LMSW) certification in NYS.
- A minimum of two (2) years of experience as a Social Worker with strong interpersonal, oral, and written communication skills; five (5) years of experience is preferred.
- Proficiency in MS Word, Excel, and Outlook is necessary.

Responsibilities:
- Provide case management and counseling services to address patients' bio-psychosocial needs.
- Evaluate patients' psychosocial requirements and determine appropriate treatment and referral options.
- Assist patients in completing forms for entitlements, supplemental insurance, and other benefits.
- Advocate for patients when interacting with community agencies.
- Maintain accurate and up-to-date patient records.
- Coordinate transportation assistance for patients as required.
- Attend case conferences and team meetings.
- Identify the need for patient education and support services.
- Conduct need assessments and education for prenatal and postpartum patients.
- Collaborate with healthcare providers and make referrals when necessary.
- Evaluate the needs of special population patients.
- Provide HIV pre-test and post-test counseling and testing.
- Follow up on missed appointments with patients.
- Participate in and contribute to Quality Improvement and Quality Assurance activities.
- Maintain a clean and safe work environment.
- Perform other relevant duties as assigned or requested.

Physical Demands:
This role may involve periods of sitting, standing, walking, bending, and stooping. Weekend and evening work may be required.

Salary Range:
$55,057 - $58,468 per year (4% increase in Feb. 2025)
